Acknowledgement And Consent Form for Employees

This form must be filled out by all new employees. It must also be filled out annually by all existing employees. This form accomplishes three things:

  • The form contains a background questionnaire concerning sexual abuse.
  • The form contains an acknowledgement that that the person has received, read, and understands the Safe Environment Policy and the Misconduct Policy.
  • The form authorizes the parish, parish school, and diocese to do a background check on the person.

Acknowledgement And Consent Form for Volunteers

This form must be filled out by all new volunteers that are required to have background checks under the Safe Environment Policy. It must also be filled out annually by all applicable volunteers. This form accomplishes three things:

  • The form contains a background questionnaire concerning sexual abuse.
  • The form contains an acknowledgement that that the person has received, read, and understands the Safe Environment Policy and the Misconduct Policy.
  • The form authorizes the parish, parish school, and diocese to do a background check on the person.

Acknowledgement And Consent Form for Independent Contractors

This form must be filled out by all new independent contractors that are required to have background checks under the Safe Environment Policy. It must also be filled out annually by all applicable independent contractors. This form accomplishes three things:

  • The form contains a background questionnaire concerning sexual abuse.
  • The form contains an acknowledgement that that the person has received, read, and understands the Safe Environment Policy and the Misconduct Policy.
  • The form authorizes the parish, parish school, and diocese to do a background check on the person.

Employment Background Check Form

This form must be filled out by all new employees. It authorizes the parish, parish school, and diocese to do a background check with the person's previous or other current employer. If the employee has been unemployed or self-employed for more than three years prior to starting employment, the person should write "N/A" in the blank labeled "Name of Previous/Other Current Employer", then sign and date the form in the appropriate places.

Response By Employer Form

This form is used as part of the Employment Background Check done on all new employees. This is the form that the employee's previous or other current employer will complete and return to the Diocese.

To prepare this form for mailing to the previous or other current employer, enter today's date in the upper-right-hand corner then enter the employee's name in the appropriate blank. This form is then mailed to the previous or other current employer, along with the Employment Background Check Form and a stamped envelope addressed to Diocese of Crookston, Attn: Background Checks.

BCA Letter

This letter applies only to new employees at parishes with schools. This letter is sent to the Minnesota Bureau of Criminal Apprehension to request a background check. The letter is completed using the following steps:

  • On the line in the upper-right-hand corner, put today's date.
  • On the lines in the middle of the letter, list all employees that you are requesting background checks on.
  • Write your phone number in the box at the end of the last sentence.
  • Sign the letter under the words "Sincerely yours,". Also print your name under your signature.
  • Make a copy of the completed letter for your files.

Once the letter is completed, the following items are mailed to the BCA:

  • Letter to BCA.
  • Copy of Acknowledgement and Consent Form for Employees.
  • Check made out to "MN BCA"-amount is $8 per background check requested.
  • Stamped envelope addressed to the Diocese of Crookston, Attn: Background Checks.
    • If you're requesting more than one background check, make sure that you have adequate postage on the return envelope. Assume that two pieces of paper will be sent back for every background check requested.

Social Security Number Validation Form

This form is used when an issue arises regarding a person's Social Security number. When a Social Security Number needs to be validated, the Background Check Coordinator will personally review the person's Social Security Card to validate that the number on the card matches the number on the Acknowledgment and Consent Form, then complete this form stating that the numbers match.
