Medical Insurance

This benefit is administered by Blue Cross Blue Shield of Minnesota. It is available to all full-time employees. Payment of premiums is negotiated between the employer and employee.

The Diocese offers two different options for its medical insurance benefit. Employees can choose between a $500 deductible plan and a $2200 deductible plan. Both plans are described below.

$500 Deductible Plan

The calendar-year deductible is $500 per person and $1500 per family. An employee will pay 100% of claims until the deductible is met. Once the deductible is met, Blue Cross will pay 80% of the next $5000 of claims, and the employee will pay 20%; this will get the employee to the out-of-pocket maximum, which is $1500 per person and $4500 per family. After the out-of-pocket maximum has been reached, Blue Cross will pay 100% of all claims for the remainder of the year.

Prescriptions have a separate copay and out-of pocket maximum. If an employee gets a prescription, you will pay $10 for generic drugs, $25 for name-brand formulary drugs, and $40 for name-brand non-formulary drugs; the remainder of the cost is covered by Blue Cross. Once you have reached the out-of-pocket maximum for prescription drugs, which is $750 per person per calendar year and $1000 family aggregate per calendar year, all prescriptions are paid 100% by Blue Cross.

If you are covered under the $500 deductible plan, you are not eligible to participate in the health savings account plan.

$2200 Deductible Plan

The calendar-year deductible is $2200 for single coverage and $4400 for family coverage. An employee will pay 100% of claims until the deductible is met. Once the deductible is met, Blue Cross will pay 100% of all claims for the remainder of the year.

Prescriptions are applied toward the deductible. An employee will pay 100% of prescriptions until the deductible is met. Once the deductible is met, Blue Cross will pay 100% of all prescriptions for the remainder of the year.

If you are covered under the $2200 deductible plan, you are eligible to participate in the health savings account plan.

Provisions That Apply to Both Plans

Preventative Care items such as preventative office calls, eye and ear exams, immunizations, vaccinations, and routine procedures for cancer screening are covered at 100% under both plans.

Eyeglasses are covered as durable medical equipment and are covered subject to the deductible and copay.

Blue Cross may pay 100% for certain over-the-counter antihistamines (for allergies) and PPIs (for acid-related stomach disorders/GERD); click here for more details.

The dependent child age limit is 19 years old, 25 if the child is a full-time student.
